Information For New Patients

If you have scheduled your initial visit to this office, we are very much looking forward to meeting and working with you to address your health needs.  As with any first-time visit to a doctor's office, there is some paperwork to take care of.  All new patients must

        --  fill out a confidential New Patient Questionnaire

        --  Read the Privacy Policy and sign the Privacy Policy Consent Form

                        Option 1:  Download New Patient Packet (includes all documents in .pdf format)

                        Option 2:  Request (by email) a fax of the New Patient Packet

                                           (include fax number and "attn: to" in body of email)

                                           to fill out the paperwork in the office.  Please call or email the office

                                           to let us know that you plan to do this.
