Forms & Financial Matters

Please download and complete the New Patient Packet and bring it with you to your initial visit.  You can also email the completed forms and/or any insurance information (a copy of the front and back of your insurance card) in advance of your appointment.  This is not mandatory, but can help us in finding out coverage in advance.

If you are unable to fill out the forms in advance, please let us know and plan to arrive about 15 minutes early for your first appointment.

A new patient initial visit includes consultation, examination, and treatment, and lasts about an hour. Follow-up appointments are roughly 30 minutes.

While Dr. Henderson is not a member of any insurance networks, Most non-HMO plans provide reimbursement for out-of-network chiropractic services, often after an out-of-network deductible is met. The office will assist in finding out what coverage is available and will submit claims electronically, but cannot guarantee that there will be insurance reimbursement. Payment is requested at time of service.  All forms of paper and plastic are accepted.

A $60 charge will apply to all follow-up appointments that are either missed or canceled on the day of service. Please call or email the office before close of business on the day before a scheduled appointment to avoid this charge.
